The following ministry forms are for downloading and the use of church members and ministry servants. Click on the form title to access and download.
Church Membership Packet - can be downloaded here or fill out online via this link. We do need a membership packet filled out by each person who is seeking membership at SGBC.
Ministry Application for Working with Children - all of our volunteers who serve in any capacity in our children's ministry must fill out a ministry application, consent to a background ground, and be trained in our specific children's ministry practices.
Ministry Application for those Working in the Nursery - this is the same application as above but includes some helpful scenarios that are specific to nursery.
Accident/Injury Report: Completed by the ministry servant if an accident occurs where an injury requires immediate attention and notification of a parent, or that results in bleeding, bruising, knot, or visible mark that remains more than a few minutes
Incident Report: Completed by the teacher when a disciplinary issue requires notification of a parent
Emergency Medical Authorization: Completed by a parent of every minor child who is attending a Youth Group event or going to camp.
